Quadrigeminy on Apple Watch ECG — what it means and what to do
Quadrigeminy is a repeating pattern where every fourth heartbeat is a premature beat. The sequence cycles: three normal beats, one early beat, three normal beats, one early beat. It is less frequent than bigeminy (every second beat) or trigeminy (every third beat).
The premature beat can be a PVC (from the lower chambers) or a PAC (from the upper chambers). ECG+ identifies which type and labels the pattern in your recording.
PVC quadrigeminy on Apple Watch ECG
In PVC quadrigeminy, every fourth beat is a wide, early ventricular beat. Three normal beats separate each PVC, so the overall rhythm feels closer to regular — many people do not notice it at all.
PAC quadrigeminy on Apple Watch ECG
In PAC quadrigeminy, every fourth beat is a premature atrial contraction. Like all PAC patterns, the early beat is narrower and less visually striking on the ECG than a PVC, but ECG+ identifies it from its timing and shape and labels it accordingly.
What quadrigeminy feels like
Quadrigeminy is frequently symptom-free. Because there are three normal beats between each premature beat, the irregularity is more subtle than bigeminy or trigeminy. When people do notice something, it is usually:
- An occasional awareness of an irregular beat, recurring at intervals
- A brief flutter or mild thump every few seconds
- Often nothing at all
Is quadrigeminy dangerous?
Quadrigeminy on its own is typically benign. Like other geminy patterns, it shares the same common triggers: caffeine, poor sleep, stress, alcohol, and dehydration. It is seen regularly in people with perfectly healthy hearts.
If it appears consistently across multiple recordings, or comes with any associated symptoms, it is worth a mention to your doctor. In most cases, the outcome of that conversation is reassurance and a plan to keep an eye on things.
What to do next
- Take recordings at different times to see whether the pattern is consistent or only occasional.
- Note any triggers — caffeine, alcohol, sleep quality, stress — and whether quadrigeminy appears alongside them.
- Share with your doctor using ECG+'s PDF export if the pattern persists or causes any symptoms.
Frequently asked questions
Can Apple Watch detect quadrigeminy?
The Apple Watch ECG app only classifies the overall rhythm and does not label quadrigeminy. ECG+ analyses the recording to identify the repeating pattern of three normal beats followed by one premature beat, and labels it as PVC or PAC quadrigeminy.

What is the difference between bigeminy, trigeminy and quadrigeminy?
They describe how often a premature beat occurs. Bigeminy is every second beat, trigeminy is every third beat, and quadrigeminy is every fourth beat. Quadrigeminy is the least frequent of the three and often the least noticeable.
